Applications should be delivered by post or in person to the CDCP SR.

Change of personal data

is possible to effect on the basis of written application delivered to the address of the central depository, whereas the change of personal data is a free of charge service. Written application must include the original data of account owner and the new (changed) data. Application must be signed by officially verified signature and verification stamp must contain the Birth Certificate Number and permanent address of account owner (verification by notary, where such data are usually included, is recommended)

Next to that the following documents must be enclosed:

  1. for change of the surname - officially verified copy of the Marriage Certificate
  2. for change of the permanent address - filled-in application with officially verified signature
  3. for change of the Birth Certificate Number - original or officially verified copy of the Birth Certificate Number issued by the Central Registry of Population Residence (address: Partizanska 129, 974 86 Banska Bystrica)

If foreign entities apply for services of depository they must submit documents in compliance with Operational rules of the CDCP.
